Currently bugged. I completed all 8 codex entries(4 from the dailies, 1 from "Tracking the Origin" and 3 from the world bosses) and wasn't awarded the 9th entry, which is the title.

This title is awarded after killing all 3 world bosses on Tatooine that are part of the Rakghoul Pandemic event. The bosses include Infected Trapjaw, Urtagh, and Zama Brak. Some players are reporting that they are not getting the title after killing all 3 bosses, but for me the achievement showed up tonight (4/17/12) as codex entry 9/9 and I received the title.

It was fixed before the end of the event. I received the title with only 30 minutes to spare before maintenance took away the event. :D
