This is not a bug its part of the fight mechanics. Wont say more so not to give spoilers but just ignore Intao's comment altogheter.

When fighting Revan at this time there is a bug if you get him below 10k HP while he is doing the ability with lightning or meteors he will bug and become immune to any attack
