To initiate this mission first time you need to talk to the droid on the Republic Fleet: Carrick Station standing by the elevator leading to Mission Departures who will offer you the mission The Jedi Prisoner. This will lead you onto the Republic Fleet: The Telos where you'll get the next stage of The Jedi Prisoner and Flashpoint: Taral V. Without doing this first you will not get the mission for the flashpoint and you will not be able to enter the flashpoint without this mission.

When you have completed the first flashpoint you will be able to pick up the flashpoint mission again from one of the smaller droids on the Republic Fleet: The Telos.

you can also get the starting quest on the Telos. the quest giver is outside oteg's chamber.

As a side note, Oteg is creepy. very creepy.
