This and the matching Main-Hand, The Heart of Ragnos, are more than likely referencing the ancient Sith Lord, Marka Ragnos.

As with all other 140 rating Offhand/Secondary weapons, the token for this drops from Hard Mode/Nightmare Mode Gharj, the second boss in Eternity Vault.

It has nothing to do with Ragnaros from WoW. It´s a Reference to Marka Ragnos, a Dark Lord of the Sith.

Marka Ragnos was first mentioned in the late 90´s in a Book, so you can say Blizzard spelt him incorrectly (and copied it). ;-)
