Got one so far: Balmorra, within the Neebray Warehouse, in the lower level, but it requires 2 people to access it, can't be gotten on your own.
The closest Flight Path would be Troida

For Sith -

As a lvl 26 Bounty hunter I used the assembly chamber on Dromund Kaas (in the western wall of the approach to the dark temple) to combine 3 crystal matrix shards in the following order (left to right) – Green, Blue, Red.

In return I got a relic for a lvl 15 with 15 aim and 10 endurance.

Inside the warehouse, go to the 2nd floor. You need 2 people to click on the controls of the force field. Others can run in with you - besides the 2 clickers.
