As of the beta weekend of 2011-11-25, the vendor who sells these is called Boibole and is located at the following coordinates:
Player coordinates: -56, 52 'ish.
Cursor coordinates: -55, 175 'ish.
She's standing outside to your right if you're walking up the stairs.

These can be selected as rewards for missions on Dromund Kaas and are dropped by various mobs, including most of the beasts in the wilderness.
For'senn is the Modifications Commendations Vendor, and is located at -33,177 on the Kaas City map. He sells lvl 9 to lvl 15 equipment mods.
Boibole is the Equipment Commendations Vendor, and is located at -56,177 on the Kaas City map. He primarily sells lvl 13 blue legs and lvl 16 brown weapons, as well as class 1 lockboxes with random contents.

Got two of these to drop in about 60 seconds while killing Kubaz Spotters in the Initiate's Path southwest of Kaas City.
