These can be found inside champion gear bags.

OR

from Battlemaster gear bags. These requires you to be rank 60 to open.

These are looted from Champion Gear Bags

You need to use Warzone Commendations which are gained from PvP zones.

1. You then use 30 of these to buy 10x Mercenary Commendations
2. When you have 200 of both you can buy Champion Gear Bags from the PvP items vendors (located on the Imperial Fleet by the PvP daily drop box, or in the PvP zones.... Republic don't need PvP items, just die Jedi scum)
3. Open bag and collect tokens.

I have not had chance to sit and see what the average amount collect is, so you will have to suck it and see :-)
It is defiantly possible to get 1000 of each token well before 50, so get saving!

Actually the Champion bags will always have 15 Centurion Commendations and 7 Champion Commendations. There is also a chance at a Champion token exchangeable for a piece of Champion gear. The tokens are specific to a gear slot type(chest, bracers, belt etc.) and class(Knight, Warrior).

Champion gear bags have chance at a champion token and centurion comms, no champion comms. (confirmed by me)
From those I've asked. the battle master bags also only contain centurion comms with a chance at battlemaster tokens. (unconfirmed by me)

I believe these are collected from the PvP Item Chests which can be purchased from the PvP Vendor by the daily mission terminal.
