I have not found any vendor that allows upgrading any lower commendations to higher ones. Was this true only in beta?

You use these at a Specialty Goods Vendor, Yerk for example.

These are rewards from various quests, but can also drop off enemies (I believe only strong or elite). If they drop off an enemy while grouped, everyone in the group can loot one off the corpse.

They can drop from any mob, but the drop rate is likely better on 'strong' or above creatures.

The vendor for items are located on the far west side of the supplies sector in Fleet

There aren't as many quests on Hutta as the other starting planets that give commendations unfortunately, so you may end up having to go out killing randomly to get the 8 needed for a chest piece or just skip it.
