To obtain this codex, you must be on the defending team, and you have to defuse a bomb while it's currently planted on the door. You have 20 seconds upon being planted to defuse it (by right-clicking). Defusing the Bomb is a 3 second cast and can be interrupted by direct attacks and interrupts, but not from Damage over Time (DoT) abilities.
