It's driving me a little nuts that the companion conversation quests aren't named until after you complete them, so as I play the game I'll be adding them here. If I find any information on loyalty requirements, I'll note that as well.

Just an FYI for all those looking for Companion Customization Kits... the only one I have seen or heard of that you can purchase for Qyzen on the "Security Key" Vendor. Which means you much get and activate an authenticator for your account, which then allows you access to that vendor.
From what I've seen and heard, it turns him a yellowish color and restores his lost eye.
